Student Health

School Nurse and Athletic Trainers

St. Pius X utilizes Magnus Health for all medical paperwork: physicals and medical action plans. This web-based system allows parents to have continuous access to your child's health record as well as the ability to  make updates when needed. The school does not accept hard copies of physicals, medical paperwork, or athletic documents.

Every student, regardless of sports participation, must have the NMAA Pre-Participation Evaluation uploaded and approved in Magnus by August 1 each school year. The physical must be dated after April 1st for the following school year.  If paperwork is not submitted/approved by August 1st, then schedules will be withheld at registration.

Annual Health Office Requirements

  • Immunization Record or Certificate of Exemption (NMSA § 24.5.2-3)
“Unlawful to enroll in school or licensed child care programs unimmunized”
    • Verification of immunization compliance or exemption from vaccination is completed annually.  Every student must have a current copy of their immunization record or approved Certificate of Exemption submitted to Magnus before attending school. The immunization records should be submitted as soon as possible to allow the nurse time to review them by August 1.
    • Students entering 7th through 12th grades must have 1 dose of Tdap given on or after their 10th birthday and the 1st dose of the Meningococcal (Men ACWY) vaccine. Students entering 12th grade must have a booster dose of the Meningococcal (Men ACWY) given on or after their 16th birthday.
    • Certificates of Exemption can take up to 60 days to be approved once they are received by the New Mexico Department of Health.  The approved certificate is good for 1 year. Please submit a copy of the approved Certificate of Exemption by August 1.
  • Dental Exam Verification Form: required of every student. These need to be updated for each school year.

Action Plans: (only required if the student has the condition):

These forms can be downloaded from Magnus, taken to the healthcare provider, and uploaded to Magnus. A healthcare provider's preferred form may be used instead of any of the following forms:

  • Allergy Action Plan
Students with severe allergies requiring epinephrine must have an Emergency Allergy Action Plan (EAAP) signed by a healthcare provider and parent/guardian. A minimum of two epinephrine injectors and the listed antihistamine should be available at school at all times.

  • Asthma Action Plan
An Asthma Action Plan is required for any student needing an inhaler during school or school-sponsored activities.
    • For non-asthma conditions requiring an inhaler, a Medication Authorization Form must be submitted.

  • Diabetes
A Diabetes Medical Management Plan, signed by the healthcare provider and parent/guardian, must be submitted and updated annually.

  • Epilepsy / Seizures
A Seizure Action Plan must be completed and signed by both the parent/guardian and licensed healthcare provider before the school year begins.  Emergency medication should be kept with the student.

Medication Authorization Form

This form acts as physician orders for the medication.  This is required for all medications administered on-campus that are not covered by an Action Plan, whether carried by the student or held in the health office.

Medications at School

No medications — prescription or over-the-counter (OTC) — may be administered by school personnel without signed parental consent documentation on file.

All student specific medications must be brought to the school by a parent/guardian and signed into and out of the health office.

  • Over-the-Counter Medications

Saint Pius X School maintains a limited selection of Over-the-Counter (OTC) medications for student use during the school day. These medications are governed by Standing Orders issued by the Department of Health.  Parents must provide signed consent or declination annually.  This is a list of covered conditions and medications. (View Here)

  • Emergency Medications
Students may self-carry and self-administer emergency medications only if this is indicated in their approved action plan.

  • Controlled Medications
Controlled substances must be stored in the nurse’s office and taken under nurse supervision. A signed Medication Authorization Form is required.

Students may not share medications with other students.

Accidents or Illness at School

All students must have current emergency contacts and medical information updated in Magnus SMR. If a student becomes ill or is injured during school hours, the school nurse (or designee) will attempt to contact the parent or legal guardian.

If unavailable, emergency contacts will be called. In cases of serious injury, illness, or inability to make contact with parents/guardians in a timely manner, Emergency Medical Services (EMS) will be contacted.

Communicable Diseases

Cases of communicable diseases will be addressed individually, considering the health of the affected individual, their contacts, and the broader school community. Information will be shared only on a need-to-know basis. When required, the school will report to the New Mexico Department of Health and Human Services and follow their directives.
