Physicals Clinic

We are pleased to offer a Physical Clinic at St. Pius X to help students complete their required physicals ahead of the upcoming school year.

Clinic Dates: July 25–26
Cost: $20 per student (card only; no cash or insurance billing)
Format: First-come, first-served — please be aware that wait times may occur due to limited physician availability.

If a parent or guardian is unable to attend, a signed authorization form must be submitted in advance to permit the student to receive a physical.

This form serves to register your student for a physical examination during the St. Pius X School Physical Clinic. Please ensure payment is made by card at the time of service. No insurance billing will be provided. All participants must arrive early to account for possible wait times.
